Ages 18-24

For youth facing the challenges of adulthood without the most basic resources or support, it can be alarmingly easy to fall through the cracks. YES’s Transitional Living Program (TLP) provides an urgently needed safety net. For transitional age youth experiencing homelessness, this 18- to 24-month program offers housing in a home-like, group setting as well as scatter-site apartments. Residents receive guided support and a comprehensive suite of services designed to enable them to become self-sufficient, live independently and successfully navigate a future without homelessness. Youth may self-refer to this program or be matched through the Youth Coordinated Entry System.

Program Goals & Objectives

The goal of YES’s Transitional Living Program (TLP) is to fully prepare older homeless youth for self-sufficiency and prevent them from becoming homeless, incarcerated or victimized adults. By developing an Individualized Service Plan and dedicating their time toward achieving their goals, whether through our Education or Employment tracks, residents are able to take full advantage of the comprehensive resources and support available to them here.
